Our Policies
We are open to serve you, answer questions or schedule an appointment during the following hours:
Monday-Friday: 8 a.m. – 4:30 p.m.
Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you may be subject to being charged an additional fee. Broken appointments without notification generate a dismissal letter from this office.
Payment Options
When you arrange your appointment, our friendly staff will advise you of the amount due at time of service. Payment in full is expected at the completion of treatment. For your convenience, we accept MasterCard, VISA, and Discover.
Insurance
We participate with the following insurance plans: Aetna DMO, Aetna Advantage Plus, Aetna Open Access or Basic Plan, Ameritas PPO, Cigna DHMO, Compdent (we do not participate in Compdent PPO), Delta Dental Premier Plan, Dentemax, Assurant (Fortis DHA, Protective Dental Care Legend or Heritage Plans and UDC), Guardian Dental Guard Preferred, Metlife PDP, and Superior Dental. We participate with Super Med Dental from Medical Mutual only at our Lyndhurst and Strongsville locations. We do not participate with Principal. However, you may be seen and use the Ameritas PPO fee Schedule per contract with Ameritas and Principal Insurance.
